The online biometric screening scheduler via Quest Diagnostics is accessible through your Vitality account. To schedule your biometric screening online, please log in to your Vitality account. Then, click Learning Center> Guide to Vitality> Vitality Points> Vitality Check> and select Quest from the Quick Links section in the upper right hand corner. The Quest link will automatically redirect you to the online scheduling tool where you should follow the prompts to complete your scheduling process.

Earn your Amplify (wellbeing) discount for 2017 insurance premiums up to $1,170/Year ($45/Pay):
- Complete the Vitality Health Review (VHR), Vitality Check (biometric screening) and achieve Silver Vitality Status before October 1, 2016 to achieve a discount of $520/Year ($20/Pay) on your 2017 insurance premiums!
- To access the VHR: Login to your Vitality account> Click ‘Learning Center’> Click ‘Guide to Vitality’> Click ‘Vitality Points’> Select ‘Vitality reviews’> Select ‘Complete the Vitality Health Review’ under the Quick Links tab in the upper right hand corner.
- To schedule a Vitality Check: Online scheduler will open May 24, 2016.
- To achieve Silver Vitality Status: Login to your Vitality account> Click ‘Points’ from the home tab > Click ‘Points Planner’> Use the ‘Quick Points Planner’ to receive a recommendation of point earning activities OR browse the ‘Categories’ section and click on activities that interest you. Then select ‘plan your points’ to save these activities for later or click ‘Act Now’ to complete activities right away.
- To access the VHR: Login to your Vitality account> Click ‘Learning Center’> Click ‘Guide to Vitality’> Click ‘Vitality Points’> Select ‘Vitality reviews’> Select ‘Complete the Vitality Health Review’ under the Quick Links tab in the upper right hand corner.
- Certify that you are a non-tobacco user or complete our tobacco cessation program and you’ll earn a $650/Year ($25/Pay) discount on your 2017 Insurance premiums.**

If you certify that you are a non-tobacco user while completing your VHR, the non-tobacco user affidavit will appear within 48 on your Vitality site for your completion. If you do not see the non-tobacco user affidavit within 48 hours, please contact Vitality Customer Care at 877-224-7117 or wellness@powerofvitality.com.
To view the non-tobacco affidavit: From your Vitality homepage> Click the gear icon in the upper right hand corner> Click consent> Scroll down to view. Once you complete the non-tobacco affidavit you cannot change your response.
- If you certify that you are a current tobacco user while completing the VHR, the non-tobacco user affidavit will not appear and instead you will be asked to consider joining the SmokeFreeTXT program. Upon completion of this program you will qualify for the non-tobacco user insurance discount in 2017 and a green check mark will appear on your personalized incentive requirement checklist, signaling that you have completed this requirement (From the Vitality homepage> Rewards> Employer Incentive Plan> Scroll down to Participate section.)

Insurance premium credits will appear in your paycheck when you meet the incentive requirements. If you were hired after July 15, 2016, you will only need to complete the VHR to qualify for the 2017 insurance premium incentive.

Know Your Numbers.
By completing the Vitality Health Review, and a company-paid biometric screening, known as a Vitality Check, each employee will receive a personalized snap shot of their current health. You will have access to an online registration tool to register for a company-paid biometric screening starting May 24, 2016.
The biometric screening provides confidential results to you about your blood pressure, fasting glucose, cholesterol, BMI, height, and weight. In the weeks following your screening you will also receive a detailed report sent to your home.
You will receive an email notification when your results are available to view online and a paper copy of your results will be mailed to your home 2-3 weeks after your screening. Your results will automatically be transmitted from Quest Diagnostics to Vitality on your behalf and Vitality Points and Goals will be awarded to you in the weeks to follow.
You will earn Vitality Points for completing a Vitality Check, as well as, bonus Vitality Points for having in-range results at the time of your screening. It is important to fast for 9-12 hours prior to your screening to ensure accurate results. Medications should be taken as prescribed. Drinking water is permitted.

The 800 Superior Onsite Health & Wellness Center provides onsite, no-cost health and wellness services to AmTrust employees through a partnership with University Hospitals. Cleveland area employees can take advantage of 15-30 minute appointments that may be scheduled at their convenience during work hours. Use of the onsite clinic for acute and chronic medical conditions is encouraged and supported by AmTrust leadership.
The onsite clinic is managed by Ellen Minerd, Nurse Practitioner, who is qualified to manage acute and chronic medical conditions, diagnose medical problems, order treatments, prescribe medications, and make referrals for a wide range of acute and chronic medical conditions.
